Home is Where the Heart is: Improving the Cardiac ABCs

McLean Home Care & Hospice is proud to join the initiative to introduce preventive cardiac best practices to the home health community. The Home Health Quality Improvement National Campaign just announced a new partnership with the Department of Health and Human Services' Million Hearts® initiative to introduce preventive cardiac best practices to the home health community.  The partnership with Million Hearts® will focus on the ABCs of heart care

  • Aspirin therapy for those at risk
  • Blood pressure control
  • Cholesterol management
  • Smoking cessation

The HHQI National Campaign will be helping home health agencies to improve and measure progress on the ABCs by creating new information and guides. They will also help agencies measure progress toward improving heart care and supporting the Million Hearts® goal of preventing 1 million heart attacks and strokes in the United States by 2017.

McLean has an innovative Living with Heart Failure Program. The program provides patients with Telemonitoring, along with self-management skills designed to reduce hospitalizations. Home care services play a major role in helping patients with chronic heart failure avoid hospital stays. McLean is honored to be among the top visiting nursing agencies in the service area for having the lowest rate of hospitalization for patients, according to “Home Health Compare” outcomes at www.medicare.gov.
